Dr. Bulosan will require a signed release form from you to authorize such family and friends unless you bring family or friends to your appointment, which would be construed as consent to such disclosure.
5. Lawsuits and Similar Proceedings. Dr. Bulosan may use and disclose your IIHI in response to a court or administrative order, if you are involved in a lawsuit or similar proceeding. Dr. Bulosan will require that the party that requests your records provide a signed records-release form.
6. Disclosures Required by Law. Dr. Bulosan will use and disclose your IIHI when she is required to do so by federal, state, or local law.
